This is an exciting opportunity for an ambitious Computing and Systems Development, Data Systems Analysis or Business Management Graduate to fast-track their career development as a Knowledge Transfer Partnership (KTP) Associate with ‘In Your Element Limited’ (Trading as Elemental Software). Elemental Software are an exceptionally ambitious award-winning Tech for Good company, helping organisations in the UK and Ireland enhance the uptake and impact of social prescribing programmes via their digital platform Elemental Core, a cloud-based transactional workflow system that dynamically connects the primary stakeholders in the Social Prescribing model.

This post is a dynamic role looking to embed capability and knowledge to support the development of a ‘Social Prescribing Population Health Management’ (SPPH) Tool, expanding Elemental Software’s analytical functionality, improving data integration and interpretation to generate robust evidence for community models of care.

Our vision for this KTP project is to inform the development of an innovative data management tool by bringing together meaningful outcome data on population based early intervention, through Social Prescribing, in order to identify and target local health inequalities and improve health across groups, communities and regions; and which will support population health management for care commissioners. The KTP Associate will be primarily based within the Product Team and work closely with the development team, sales team, and wider stakeholders to learn and document requirements, contribute findings to user manuals, and ensure that any new technical requirements are properly integrated with existing processes.

As the social model of health gains significant momentum, housing associations, prisons and the local authorities are showing an interest in adopting this model of care; to facilitate population health management. To meet this need you must gain a thorough understanding of what data to link/combine; health behaviours and lifestyles, health status, welfare, housing and fuel poverty, alongside the availability of services to determine social, economic, and cost-effective intelligence.
